All parents are asked to return the federal card forms that were sent home to all students last week. Deadline is Tuesday, November 10th.

Please answer the 3 questions by marking the appropriate "yes" or "no" boxes, completing the additional information requested for any "yes" answers, and then sign and date it. 

Note:  Even if all of the answers are "no" it is very important that the card be returned by Tuesday, November 10th!
